Community Integration Specialist (CIS) for CDS

Community Integration Specialist (CIS) working in Consumer Directed Services (CDS) is a person-centered role focused on helping individuals with disabilities or senior citizens lead independent lives within their own communities. Unlike traditional facility-based care, CDS allows the client (consumer) to direct their own care, and the CIS acts as a bridge to community resources.

Job Summary

The CIS is responsible for designing and implementing individualized plans that foster independence, social inclusion, and skill development. You will work directly with consumers to identify personal goals and connect them with local opportunities for employment, education, and social engagement.

Core Responsibilities

  • Individualized Planning: Develop and document goal-oriented support plans tailored to each consumer’s unique needs and interests.
  • Skill Building: Provide direct instruction in daily living skills, such as money management, social communication, and using public transportation (travel training).
  • Community Advocacy: Identify and secure accessible community resources, including volunteer opportunities, recreational programs, and non-paid social networks.
  • Safety & Compliance: Monitor consumer progress and safety during community activities, ensuring all services adhere to state and agency regulations.
  • Collaborative Care: Work with family members, healthcare providers, and case managers to ensure a cohesive support system for the consumer.

Key Qualifications

  • Education: High School Diploma or equivalent required; an Associate or Bachelor’s degree in Social Work, Psychology, or a related field is often preferred.
  • Experience: Typically 1–2 years of experience in social services, particularly working with individuals with intellectual or developmental disabilities.
  • Skills:
    • Strong interpersonal and verbal communication skills.
    • High degree of empathy, patience, and compassion.
    • Exceptional problem-solving abilities to navigate barriers to inclusion.
  • Technical: Proficiency in basic documentation and report writing (e.g., Microsoft Office or Google Workspace).
  • Licensing: Valid driver’s license and a reliable personal vehicle for independent travel are usually mandatory.

Physical & Work Environment

  • This role is highly mobile, requiring travel to consumer homes and various community locations.
  • Must be able to perform physical tasks like walking, standing, or assisting with mobility devices.
